The following chart lists the services and supplies covered under the hearing benefit for the Trooper Chapter, Airport Police and Fire Officers Chapter, Fairbanks Police Department, Fairbanks Fire Department and Juneau Police Department members, as of July 1, 2009. Refer to your Benefit Booklet for details, general limitations and exclusions

100% after $25 copay; one exam every two years. |
| Hearing Hardware |
80%; maximum benefit of $3,000 during any 3 consecutive calendar years. |

Covered services |
- Ear exam
- Hearing exam and testing
- Hearing aid prescribed as a result of the examination
- Hearing aid rental while primary unit is being repaired
- Ear mold(s)
- Hearing aid instrument
- Initial batteries, cords, other necessary ancillary equipment
- Warranty
- Follow-up consultation within 30 days following delivery of the hearing aid with either the prescribing physician or audiologist
- Repairs, servicing and alteration of hearing aid equipment

60% and subject to the deductible (does not apply to Alaska Heritage Select plan). |
| Limitations |
In order to receive benefits:

You must be examined by a licensed physician (M.D. or D.O.) before obtaining a hearing aid, and
-
You must purchase a hearing aid device. |
